Hazards Screen

Hazards are the starting point for creating and managing BowTies. Bowties are usually used to manage high risk hazards that may have been identified in a Hazard Register.

The Hazard screen is the gateway to the BowTie diagrams. At the top of the screen is a drop-down list which allows the Case to be changed. Below this the screen is laid out as a tree view with the Locations and then the Hazards that belong to each location. There may also be sub-locations and then further BowTies.

Each BowTie is defined by three key elements

  • Location – Where the activity or risk occurs
  • Hazard – the activity you wish to perform but requires management
  • Top Event – the unwanted event that could occur.

The BowTies are indicated by a circle icon on the left hand side🔴. The colour of the circle is determined by the Hazard Type. Hazard Types can be configured in the Admin section. On the right hand side of these rows, there are two icons.

  • 👁️ Hazard Overview icon – This will navigate you to view the Hazard in the Hazard Overview screen.
  • 🔴 BowTie Diagram icon – Opens the Hazard in the BowTie Diagram.

Above each location is the option to “New AI BowTie” (if this option is enabled for you) or “New BowTie”. Both of these items will allow you to create a new BowTie diagram.
